Tócate la nariz: es firme y musculosa. Ahora tócate los labios: son suaves y elásticos. El cuello de tu útero pasará de ser firme como tu nariz a ser suave y elástico como tus labios. Al final de esta fase temprana el cuello del útero se ha dilatado tres o cuatro centímetros.

Si bien la experiencia es diferente para cada mujer, es común que el parto comience con contracciones cada diez minutos, con una duración aproximada de 30 segundos cada una. Hacia el final de la fase temprana, te vendrán aproximadamente cada cinco minutos y durarán entre 40 y 60 segundos cada una. Algunas mujeres tienen contracciones más frecuentes durante esta fase, aunque tienden a ser suaves y a durar menos de un minuto.

Puede que también notes un aumento de mucosidad en el flujo vaginal, que a veces va acompañada de algunos hilos de sangre. Es lo que se conoce como el tapón mucoso (el tapón que sella el útero durante todo el embarazo). Esto es perfectamente normal, pero si notas más de unos hilos de sangre, llama a tu médico o comadrona. Comunícate también con tu médico si rompes aguas (rompes la fuente o la bolsa), incluso si aún no tienes contracciones.

Qué puedes hacer tú


Por lo general las contracciones de la fase temprana son soportables. Probablemente podrás entretenerte haciendo alguna cosa en la casa. Y tal vez incluso sientas deseos de caminar un poco. Si en cambio sientes ganas de relajarte, date un baño caliente, mira una película o dormita un poco entre cada contracción si es que puedes. Asegúrate de tomar mucho líquido, de manera que estés bien hidratada, y de comer algo. Los alimentos ricos en carbohidratos, como el pan, las papas y la pasta, son los mejores ahora. Y no te olvides de ir al baño con frecuencia, incluso si no sientes muchas ganas. Si la vejiga está llena puede resultarle más difícil al útero contraerse eficazmente y si la vejiga está vacía el bebé tendrá más lugar para descender.

El parto activo es cuando las cosas empiezan a acelerarse. Las contracciones se vuelven más frecuentes, más largas y más intensas, y el cuello del útero comienza a dilatarse más rápidamente, hasta alcanzar 8 centímetros de dilatación. Al final de esta fase puedes tener contracciones cada tres o cuatro minutos y cada una puede durar entre 60 y 90 segundos.

Qué puedes hacer tú


Como regla general, una vez que hayas tenido contracciones regulares y dolorosas cada cinco minutos durante una hora (cada contracción con una duración de alrededor de 60 segundos), debes llamar a tu médico o comadrona y dirigirte al hospital o centro de maternidad. Algunos médicos prefieren que llames antes, por lo que deberás aclarar esto de antemano.

Quizás sientas que las contracciones llegan juntas, una después de la otra. Trata de escuchar a tu cuerpo. ¿Qué te está pidiendo que hagas? ¿Estarías más cómoda en otra posición? ¿Necesitas algo de beber o de comer para que te dé energía? ¿Te ayudaría ir al baño? ¿Necesitas que tu doctor o tu comadrona te ofrezcan más información sobre el proceso del parto?

Los ejercicios de respiración y las técnicas de relajación te pueden ayudar mucho en este momento, y tu compañero/a de parto (sea tu pareja, tu mamá o una amiga experimentada), puede recordarte cómo hacerlos. También puedes pedirle que te dé un masaje en la espalda o en los pies.

Considera la posibilidad de darte una ducha o tomar un baño, porque el agua caliente ayuda a aliviar el dolor de las contracciones. Algunas mujeres se meten en la tina con una dilatación de cinco o seis centímetros y una hora después se han dilatado por completo.

Algunas veces el proceso de dilatación va muy lento o se para de repente. Dar un paseo por el pasillo del hospital quizás te ayudará. A veces un buen llanto te ayuda a relajar la tensión, así que si sientes ganas de llorar, no te prives de hacerlo.

Quizás el doctor o la comadrona sugieran romper tu bolsa de aguas, si todavía no se ha roto naturalmente, para acelerar el trabajo de parto. Ten en cuenta que después que se rompe la bolsa las contracciones sueles ser más duras.

Si te está costando mucho soportar las contracciones y no hay nada que parezca aliviarte, y deseas usar medicación contra el dolor, ahora es el momento de hablar con tu médico o comadrona y pedirle que te administre medicación para el dolor o que te aplique una anestesia peridural.

Durante la transición el cuello uterino se dilata completamente de 8 a 10 centímetros. Esta es la parte más intensa del parto. Las contracciones son generalmente muy fuertes, aparecen alrededor de cada dos minutos y medio o tres y duran un minuto o más. Puede ser que empieces a temblar o tiritar, aunque esto no les sucede a todas las mujeres.

Cuando el cuello uterino se encuentre totalmente dilatado y ya se haya completado la transición, por lo general el bebé habrá descendido un poco por la pelvis. Aquí es cuando podrías comenzar a sentir presión sobre el recto, como si tuvieras ganas de ir de vientre. Algunos bebés no descienden de manera significativa hasta más tarde, en cuyo caso la mamá puede llegar a dilatar totalmente sin sentir ninguna presión rectal. Es diferente en cada mujer y en cada parto.

Si te aplican una peridural y quieres participar activamente durante la etapa de pujar, pide que te bajen la dosis al final de la transición.

Qué puedes hacer tú


Aférrate a la idea de que ya casi ha terminado todo y pronto tendrás a tu bebé en los brazos. Aprovecha el ratito que tienes entre contracción y contracción para descansar y relajarte. Encuentra la posición que más te ayude, pujar, durante las contracciones. Respira rítmicamente, inspirando por la nariz y espirando suavemente por la boca. Si tienes ganas de gruñir o gritar, ¡hazlo!

Una vez que el cuello uterino está totalmente dilatado, comienza la segunda etapa del parto: el descenso final y el nacimiento del bebé. Al comienzo de la segunda etapa, las contracciones pueden ser un poco más espaciadas, lo que te permitirá descansar entre ellas. Para muchas mujeres las contracciones de la segunda etapa son más fáciles de sobrellevar que las del parto activo porque el poder pujar les proporciona algo de alivio.

Con cada contracción, la fuerza del útero — junto con la fuerza de los músculos abdominales si estás pujando activamente — ejerce presión sobre el bebé para que continúe descendiendo por el canal del parto. Cuando la contracción se termina y el útero se relaja, la cabeza del bebé sube un poco otra vez. Avanzará “dos pasos hacia delante, y un paso hacia atrás”. ¡No te desesperes! Con tal de que vaya quedando un poquito más abajo con cada pujo, están progresando. Prueba diferentes posiciones para pujar hasta que encuentres una que sea cómoda y te resulte eficaz.

Cuando la cabeza del bebé esté ya bien abajo y empiece a estirar tu vagina, quizás sentirás una sensación de ardor o comezón. El momento en que el pelo y la cabeza están bien visibles se llama coronamiento. Es un momento muy emocionante y un signo de que el final se aproxima. Puedes pedir un espejo para ver al bebé por primera vez o tal vez simplemente desees extender la mano y tocar la parte superior de su cabeza.

Quizás tu médico te pida que pujes más suavemente o que dejes de pujar para que la cabeza del bebé tenga la oportunidad de estirar gradualmente tu vagina y perineo. Un parto lento y controlado te puede ayudar a prevenir el desgarro. En esta etapa, la necesidad de pujar puede ser muy fuerte, de manera que recibirás ayuda para respirar o jadear durante las contracciones para que puedas aguantar sin hacerlo.

La emoción en la sala crecerá a medida que la cara del bebé comience a asomarse: su frente, su nariz, su boca y finalmente su mentón. Luego su cabeza girará hacia el costado a medida que los hombros rotan dentro de tu pelvis para colocarse en posición para salir. Con la próxima contracción, te dirán que pujes a medida que salen los hombros, uno por vez, y después el cuerpo.

Si ya has tenido un bebé antes, la segunda etapa del parto puede durar sólo cinco o diez minutos, pero si es tu primer bebé, puede durar varias horas.

Qué puedes hacer tú


Escucha tu cuerpo y puja cuando sientas la necesidad de hacerlo. Trata de no aguantarte la respiración mientras estás pujando. Puja tanto tiempo como quieras. Y vuelve a pujar. Verás que pujarás varias veces con cada contracción. Usa la fuerza de la gravedad para ayudarte, levantándote, arrodillándote o poniéndote en cuclillas. Si estás muy cansada y quieres echarte, recuéstate sobre el lado izquierdo. Esto deja más espacio para el bebé que si estás recostada de espaldas o medio sentada en la cama.

Si te han puesto una peridural, tu doctor te dirá cuándo debes pujar.

Pocos minutos después del alumbramiento, el útero comienza a contraerse nuevamente. Las primeras contracciones generalmente hacen que la placenta se desprenda de la pared del útero. Cuando el médico observe signos de desprendimiento, te pedirá que pujes suavemente para expulsar la placenta. Por lo general, se trata de un pujo corto que no es difícil ni doloroso.

Tu doctor la examinará cuidadosamente para cerciorarse de que ha salido entera. Quizás también quieras echarle un vistazo al órgano que ha ayudado a tu bebé durante todo el embarazo.

En promedio, la tercera etapa del parto dura aproximadamente de cinco a diez minutos, aunque puede llevar hasta 30 minutos o más.

Qué puedes hacer tú

Probablemente ni te darás cuenta de lo que está pasando durante la tercera etapa, porque toda tu atención estará puesta en tu pequeñín recién nacido. Ver y abrazar a tu bebé y amamantarlo estimulará la producción de hormonas que ayudan a que la placenta se separe.

Ahora que el parto ha terminado puedes sentirte temblorosa y exhausta, o llena de energía y lista para cargar a tu bebé y bailar con él por toda la habitación. Algunas mujeres, sin embargo, están tan cansadas que no pueden concentrarse en su bebé. Esto no quiere decir que sean malas madres, solamente que necesitan descansar un poco. Otras sienten un hambre feroz, así que no olvides llevar contigo al hospital algunos refrigerios por si acaso.

Admira a tu bebé, cuéntale los deditos de las manos y de los pies, sujétalo bien cerca de ti, desnudito contra tu piel, y ofrécele el pecho lo antes posible. No todos los bebés tienen ganas de tomar el pecho a los pocos minutos de haber nacido, pero mantén sus labios cerca de tu seno durante un ratito. La mayoría de los bebés comienzan a amamantar aproximadamente durante la primera hora después de haber nacido si se les da la posibilidad. Dar el pecho pronto es bueno para el bebé y puede ser altamente gratificante para ti. Más aún, el amamantamiento hace que tu cuerpo libere oxitocina, la misma hormona que causa las contracciones y ayuda a que el útero permanezca bien contraído.

A menos que el bebé necesite atención especial, insiste para que los dejen estar juntos y tranquilos un rato. El momento de darle gotas para los ojos y vitamina K puede postergarse un poco. Seguramente tanto tú como tu pareja querrán compartir juntos esta ocasión especial en la que se familiarizan con su nuevo bebé y aprecian el milagro de su nacimiento.
